ADD/DROP CLASS FORM

If you are currently registered with our studio, please fill out this form if you would like to add/drop a class, add a sibling, or unenroll.

When do I need to fill out this form?

  • I am adding a class to what I’ve already registered for

  • I am dropping a class from what I’ve already registered for

  • I am pulling my student from this program.

  • I’m adding a sibling to the program

Please note that all changes must be made before the 20th of the month in order to apply for the following month. For example, if you would like to drop your child from a class in March,  you must notify us by February 20th or else March fees would be assessed to your account, which you will be liable for.
